7.5 kV is somewhat not a surprise; 20 : 400 transformer winding ratio is more than enough to allow this to occur, in reality it also depends on the transformer core materials. Naturally, it also depends on current (ampere) which flows through the transformer primary winding. The schematic also kind of looks like it is designed to operate as an igniter.

Transformer manufacturing is somewhat more of a black art than anything else, much more pronounced with high frequency switchmode transformers.
