Track the little bubbles on the wires around the transistor PNP you will find out why

Thanks hurz, Im scrapping this design for a more efficient one. This one has a risk of loosing the Op Amp or blowing my power supply. Thanks again.

You mean blowing your transistor? I think the transistor is at a MUCH higher risk than your power supply.
