The input is one unbalanced source, and the output is a balanced audio source. Like taking a guitar signal from a 1/4 inch TS connector and making it balanced on the output (TRS or XLR). A similar circuit is done in a direct box, but that is done with transformers to keep everything passive. This circuit might be seen in an active direct box, although the balanced outputs would be much lower voltage.
