Verified, designed for 9v but potentiometer can allow adjustment to 12v. Use LM339 or separate op-amps / comparators and 1N4733A 5.1v zener diode.

Good circuit, don't batteries have a lesser usable voltage range?

It was designed for PP3 alkalines, they hit the cliff at 6 volts. http://www.powerstream.com/9V-Alkaline-tests.htm The idea was to show 9, 8, 7 and 6.5 volts. You can narrow the range by reducing the values of the 1.8K resistors and then re-calibrate the potentiometer.
