This is a BPF Multifeedback equalizer. I show three bands of the five it should have. They are 250 Hz, 1 kHz and 4 kHz from top to bottom.
It has a quality factor of 0.67 and a gain of 0.53.
The volume controls are the potentiometers at the right of the diagram.
