The voltmeter measures the error between the + & - inputs: it is the vd which gets amplified in the differential stage (see my previous circuit) making the output (-) following the input (+).

To see the high open-loop gain break the loop, connect the inverting input prepared and observe the output saturating fast (not the voltmeter but where the loop starts before breaking).
