Input Signal Conditioner - Attenuator - Superimposer

I am designing this circuit to handle a large amount of input voltage (+/-) and output it as a linear, attenuated and Superimposed signal that ranges from 0-5V (actual range is slightly smaller). It can handle 48V (96Vpp).
