Reduce your duty cycle. The 95% ish continuous duty cycle isn't going to do you any good. Your charging up the inductor (and maybe even saturating it), but your not allowing it to discharge into the output capacitor.

In fact your duty cycle looks like it's at 99%. Not going to do much for you
