As far as I can tell, I don't think the transitor and the pulsed supply are actually doing anything other than simulating a high pwm signal. You might be better off just connecting the pot straight to the 12v supply (and disconnecting one side of the pot so it only goes low to high and not high-low-high).

Well the whole point of this circuit is to have a motor spin at a very low speed

You can still do that with just resistors, the higher the resistor the more the current to the motor is reduced (which is pretty much what your pot is doing). Downside is the resistor dumps a lot of energy as heat so it's not very efficient. https://everycircuit.com/circuit/4641089417641984
