Circuit from the 'Phat EQ II' by Ibanez, as you can find it in a GSR200 bass guitar.
A little confusing layout but works well this way and a good starting point for custom designs.
Op-Amp is TL072, or any similar dual op-amp.
The green node at the 10uF capacitor is connected to the Vcc+ input of the op-amp.
