I see this is your first circuit, so one thing I noticed, When switching there is a spike of amps that places an x trhrouh the LED indicating that twice the nominal amperage for the LED of 20 ma was exceeded, thus burning out all the LEDs at once. A current limiting resistor placed in series with the LEDs by the switches would prevent a spike from damaging the circuit.
