The opamp input must be swapped or you run into a rectangular generation as schmitt trigger. “100 rms“ suppose to be just 50Watt in this +-20 setup, but with +-45V supply from math point of view 250Watt (rms) @4Ohm is possible.

You have made changes here and there but the biggest issue is from 260mV peak visible cuz the controlling opamp input has to be swap input terminals +- or its a schmitt trigger and not an amplifier

You create more and more nonsense, check your latest input circuit i separated but did not modified, check its schmitt trigger behaviour caused by positive feedback. This is not a linear preamp stage as long you mix +-input terminals. Check this http://everycircuit.com/circuit/4564608554893312
