Is it possible to build an astable MV with the two collectors connected and the load on the emitter? I want to build one with lightbulbs and to-3 transistors and I want to only use one heatsink if possible.

You need some voltage gain which a voltage follower does not has. Put both transistors on one heatsink and use some mica to isolated both collectors. Anyway, 30mW is not much power were a heatsink is needed.
