The output power is 4.8W and the draw is 13W. An efficiency of 36% is reasonable.

Just slight improvement. The Vpk-pk is 130V meaning that the Vmax is 65V, not full 120V swing. This way the output power is 16.5W and the drawn power is 36W leaving you with 45% efficiency. I suggest you seek a way to further increase the output Voltage, for a full 120V-0V-120V AC, and bring it down to the original frequency of 60Hz. At 220Khz many digital devices may misbehave of completely fail, plus there will be significant EMI, for you to filter. Just give it a few more shots. I'm sure you'll get it right.

Still only 60Vmax into the load. You're having a 120Vpk-pk and it should be 240Vpk-pk

Even for a 120 VACRMS signal? Pk on that is only 170 no?
