The 1uF capacitor is to keep the circuit from starting with a large DC current going through the transformer windings which will cause problems. This circuit’s output has less than 10nV of noise at 360Hz.

Love this circuit; I've never had the opportunity to play with any power electronic designs, but this seems so useful!

All you have to do is send the AC power through a step down transformer to provide a more reasonable voltage range and then send that through a full wave rectifier and a smoothing capacitor (1mF in this case) where it then feeds the collector of a regulating transistor which then provides your output. It’s up to you to find a way to further smooth the output from the full wave rectifier until suitably smooth and feed that to the base of your output transistor. The way I smooth the output from the full wave rectifier is by sending through a constant current source (right PNP with 100uF capacitor and 100kOhm resistor) which then feeds into a zener diode for further smoothing which then goes into a single stage RC network for further smoothing. In this case I use a power PNP controlled by the NPN to provide more power with no loss to the quality of the output.
