Take a good look at your output. Is isn't a "smooth 5V" output. Its 3.3V with ripples. You don't even have a regulator.

mains peak voltage is 339 v.. 220 or 240 v is the RMS value. So the voltage source need sorting out. Then the output voltage might get nearer to 5 volts.
