The output is 18V at 18A. You have to measure the differential potential through the load. Add a voltmeter in parallel with the load.

Where arw those "5.5 kV" flowing? Voltage always exists between two points that are directly connected, that's why in an open circuit no voltage will flow!

Yes the potential difference between the two nodes are not 5.5kV but around 18V

It is 5.5kv from highlighted node to center tap ground, its a series rlc. However connect the 5.5kv through 1k ohms through ground and then we see nothing there.

I've never heard of voltage "flowing". In an open circuit, no current will flow. If you try to measure voltage between two points that are directly connected, you will read zero.
