ripple is the last thing, more important thing is to say there is not any galvanic isolation and the maximum current it can flow to the load is just few milliamperes. And 220 are rms, 319 peak set in EC

Use bigger smoothing capacitor and insert rs resistor in front of the zener diode approx 100R

Similar to the circuit found in the philips dubai lamps, only I think they have a tranistor regulating the current instead of the zener for voltage.
