Why is there square wave when input signal is 3v and sine wave when input signal is 1v ?

Because the cut off value for the opamp is +-12. Change it to 15 volts and it'll turn into a sine wave. There is a equation for that.

Yeah. It's not really a square wave, just a clamped sine wave.
