Increasing the feedback resistor above 6K causes the output to change to square wave.

The maximum output of the op amp is set to +- 15v. If the gain's set too high, the output will try to swing above/below this. People say it's "hit the supply rails".

Yes, this is a cutoff distortion, and also your op amp is not set up correctly. This is set as an inverting voltage amplifier, but with reversed terminals. The - goes to the signal source

This is a "noninverting schmitt trigger" setup. Nothing else.
