Many things to comment... Input stage is wrong, you just use the base emitter diode, test that by removing both collector wires to the upper NPN transistor which suppose to be a current source but is in best case a voltage follower. A current source must be placed below the differential amp at thier emitters. Voltage adaptation stage is also unconventional and cause an extreme high quitsense current at push pull out stage.
