How come there's 20mV at the collector, even though the switch is open?

it's voltage forming on the N-P junction while current flows through the base and emitter. You can see it because the collector is floating. Adjust the reverse gain and the voltage will change

Remember, a transistor is a replica of two diodes opposing(NPN) or imposing(PNP), and the voltage is measured upward, but it drops down to 20mV from the base to the emitter.
