If you put a resister and capacitor in series and connect ground to the free end of the capacitor and connect the free end of the resistor to the output of the rightmost op-amp then you would have a class-D amp with the output in between the resistor and capacitor.

You shouldn't need the bottom Opamp. Just offset the sine wave with resistors. No need to buffer it.
